Lucca sells their own pizza dough, small bags of Caputo 00 flour, and canned tomatoes, as well as a great selection of cheese and meats.
Molinari sells a great selection of cured meats and fresh sausages. They also have canned San Marzano tomatoes and a selection of cheeses.
Bi-Rite generally carries one type of 00 flour, although the brand sometimes changes. They also have some excellent (and not relatively inexpensive) buffala mozzarella as well as some Molinari meats and excellent vegetables.
Rainbow carries bulk 00 flour. They've recently switched from Cauputo to an organic brand. The bin is often empty though, so get there early. They also carry some good cheeses and have canned San Marzano Tomatoes.

Ferrari sells their own pizza dough, and has a selection of cheeses.
Mollie Stones is one of the few shops that carries Grande brand Mozzarella.
The Potrero Whole Foods also sells Grande Mozzarella.
They used to carry Caputo 00 flour but don't seem to still have it.
